Which Form used to list lawsuits and potential lawsuits?

I currently have a few lawsuits against me, unrelated to any collateral. I also have multiple potential lawsuits for people I've done work for that may sue me (it's a long story but I referred some people to a lender who went belly up and they are suing everyone involved...for hundreds of thousands of dollars.)

My bank account was frozen, I have no money, and need to file this on my own (with the help of a non-profit agency).

Question: What form do I use for listing lawsuits/complaints filed and also for lawsuits that may be filed in the future? I want to list anyone that I ever referred to this lender.

Any help would be greatly appreciated.

Statement of financial affairs, line 4 is where you put lawsuits.

After you get your BK notice, be sure to send a copy to the clerk of the court(s) where you are being sued and have it entered in your case(s).
