Name Change Question - After Marriage

Helro...odd question to put on the BK forum I know but I dunno where to ask this and I can't remember how I did this in my first marriage. After my first marriage and having to change my name BACK to my maiden name, and the incredible hassle it was I vowed I would never change my name again....

I got remarried in June of 09 and had no intention of changing my name. On our marriage certificate/license it just has my full name and my husband's name. However, he now wants me to put his name somewhere in my name...

I have said that I will add a second middle name with his last name to my name.... So, it would be FirstName MiddleName Vork (his name) and then my current LastName. I was thinking I would do it this way so I don't have to change any bank accounts or credit cards work etc...

So, my question is... how the heck do I do this? I looked on my first marriage license and it has my name and my ex husbands name listed exactly the same way my current husband's and mine are listed on our marriage license... but everywhere I look online it says that my marriage certificate needs to have my married name on it... and I know my marriage license from my first marriage did not have my married name on it but I was still able to change it.

So, does this matter? Can I just use the marriage license as evidence that the marriage occured and then change my name with social security anyway that I wish? Or do I actually have to get a court order to change my name to the way I want it?

You go to your county circuit court (or check their website) to get the Petition for Name Change. Fill it out, file with the court, wait for them to approve it, voila!

eta: I believe the forms have to be notarized, etc. But all the info should be able to be found on your county circuit court's website
Filed pro se, made it through the 341, discharged, Closed!!!

Ok thanks... I don't recall having to do that before, but I'll check it out.
