    If you are using Bankruptcy as the context to remove the judgment, then you need to do this in the Bankruptcy Court, not the local Circuit Civil court. Additionally, you will probably not find any template or local form for this. It's a Motion to Avoid Lien.

    Exactly what are you trying to do? If you're trying to avoid a non-consensual (judgment) lien, then you would prepare and file the Motion to Avoid Lien in your bankruptcy case. You would follow all the local rules for noticing, and scheduling a hearing on the matter. If you can use "negative" noticing, then you would use that as well (meaning that you may not even need a hearing). In most cases, the creditors do not fight this, so that's why negative noticing could work for you.

    Please note that filing a Motion to Avoid Lien under 11 USC 522(f) requires a very specific formula that you must show in your pleading. You may be able to find "sample" pleadings (Motion to Avoid Lien) through your District or another District since the pleadings are similar.
