Statement of Fin Affairs Question Form 7

I am married and filing individual ch 7. Do I need to list my wife's income on Form 7? I know I need to do it on Schedule's I, J and the means test. But for form 7 #1 income for the past 3 years, Should I use my return number our our Joint #'s? Also same from line 3, Since I am only a co-signer on my wife's car, do I need to list that as payments made?
