Filing Chapter 7 and have questions

I am filing Chapter 7 and having someone prepare the papers and have a couple questions

1) I have collection accounts with one company. I want to make sure that even though they have different account numbers as long as they have the same name and address I just put them down one time and the total balance of all account numbers.

2) I called a hospital that I owe a very large amount to. Some of the accounts have been sent to collections already and some still are with the hospital. They told me that if I pay the entire amount they will inform the collection agency that the balance has been taken care of. So do I put down the hospital and the total balance or do I need to put down the hospital and the balance at the hospital and then put the collection agency and the balance with them? I could not get the lady to give me a balance of just what is still with the hospital and what part has been sent to collections.

Any help would be great.

I would list each of the separate accounts separately on the schedule, but list the creditor ONCE on the matrix. If there are collection agencies involved, then list the original creditor and account, and list the collection agencies as "also notify".

Also, where you are not sure of amounts, you can even put $1 for the amount. It is up to the creditor to file a claim if they want to get a disbursement from the bk trustee (if there is any to be made).
