On the proof of service form there is a section stating that I am over 18 and not a party to this action. Do I have to have a third party serve this form? I am confused.

If your jurisdiction (District) requires service to be performed by someone other than a party to the bankruptcy, then you, the debtor, can't serve it. That means even put it in the mail. It must be done by a third party.
You could get your child, friend, neighbor, family member, all over 18 years old, to sign the certificate of service and for them to place it by their own hands into the mail.

Thank you. I am in Florida, and I filed pro se 3/1/10. I am filing by myself.
I had a notice of proceedings sent out by my trustee come back to me return to sender.( She put my return address on it). So I had my husband sign the proof of service and send it certified mail. I made copies of the certified mail stub and receipt, and sent them along with the proof of service form and a copy of an amended matrix with the new address that I sent it to along with an address of a collection agency for the company to both the clerk and my trustee. Do you think that I covered all of my bases?
