On creditor matrix, how to list debts?

I'm confused on how to list some debts that are with a collection agency, lawyer offices.

For example, my Discover Card is now with Bishop, White, and Marshall law offices. Just about everything now has a collection agency attached to it.

How do I list this? I was thinking that I only list the original creditor on the matrix, but then informally sending any secondary collectors collecting on the original collectors behalf my case number in case they don't get the word from the original creditor?

On my case, the original creditor was listed with the dollar amount. All collection agencies were listed "for information purposes only" with $0. as the debt.

      Exactly what Frogger wrote.

I wrote this for the duplicates and collection agencies... "for notification purposes only". I also put $0 for the amount and I checked the "disputed" box as well.

I listed the collection agency right below the original creditor, and wrote "Collection for (original creditor)" with an amount of $0. It's important to have an amount of $0 so it doesn't get added twice.

The directions for filling out Schedule F that I printed out as reference, says that I have to also list the date the claim was incurred (are they talking about "date of 1st delinquency" as listed in my credit report?) and what it was for (i.e. goods purchased, extension of credit, medical treatment, etc.). So does anyone have an example of how they typed up their creditors matrix with all that? Like am I supposed to type mine out like the following?

Time Warner L.A. Southern Division, $140
            February 2005, cable service
Account (last 4 of my SSN here)
            see CMI below

            CMI, Credit Management Inc., $0
            July 2005, cable service
            4200 International Pkwy
            Account (acct # here)
            Carrollton, TX 75007-1912

Also, I'm in Ohio, and I just saw directions posted online to upload a text file of creditors matrix to a website. I think that's how they do it here, instead of a disk or something?

Schedule F and the matrix are not one in the same, although the matrix is derived from schedule F. (and the other schedules where you list debts, as well.) The MATRIX is a mailing list. Only addresses go on there. Be sure you understand the formatting that your district requires. You will have to re-download the matrix to show the current one from PACER every time you have to serve the matrix with something and they will compare to be sure you sent to the same addresses.

I used dates from my credit reports for the dates incurred.

Most of my unsecured debt was either medical providers or medical collections, and I annotated them as such. There were a smattering of others that I listed as consumer services, or what they actually were... cable bill. phone bill. whatever.

I don't know about uploading the text file for the matrix. I wish I was allowed to file electronically, it sure would save me a lot of time. I printed mine according to the instructions in my district and mailed it in.

My local rules said to provide a CD, but actually the clerk said they didn't take them so we were both glad I'd brought a hardcopy "just in case". Thus, if possible, maybe give them the option, or, better still, ask the clerk's office if the rules are unclear. (Actually, digging around our court's website, I found mutually contradictory instructions on mailing list format.)

                  new question re: creditors matrix

I have three accounts with one creditor (a hospital). Do I list the creditor three times with each account # in each, or do I list the creditor once, and list all three account #'s as one entry?

                  i.e.

                  City Methodist Hospital
                  # 123456, # 234567, # 345678
                  1120 Hospital Way
                  Anytown, OH 41234

I did the latter and nobody expressed displeasure.

The important thing is just to make sure that all the information is there and easy enough to peruse. For instance, listing the creditors alphabetically, to help them find themselves in the schedule.

I also listed one account like this. I know I have several account numbers with them, but they figured it all out and came up with a balance for their proof of claim.

                        Proof of Claim?

Proof of Claim is just for Ch.11 BK's, right? I pulled up a form online to look at, and it referenced Ch.11.

From the sig, you'll see tigergem's chapter 13. Even chapter 7 creditors may file proofs of claim if they think there'll be any assets to get a piece of.

All Chapters of Bankruptcy may require claims to be filed. A creditor is not "required" to file a claim because it submits them to the jurisdiction of the Court.

                            Chapter 7, 9, 11, 12, and 13 all have a claims process.

                              Blonde Moment

Well I didn't see tigergem's sig, so call it a Blonde Moment or whatever. I have no assets for any creditor to get a piece of, so hopefully my Ch.7 will go smoothly. I'm still working on the forms & creditors matrix, and hope to file pro se later this month. Fingers crossed!
