It asks for the date the claim was incurred, which to me is like asking when did you open that credit card account. What if I don't know the exact date?

You should generally put the last time you incurred major charges on the card. So, if you used the card in January 2009, then you put January 2009. However, most credit cards accumulate their balance over time. I still put the last date of a charge on my Schedule F.
