    Social security card

At the 341, does one have to bring the social security card? Can I bring a copy of an old SS statement instead?? It has my number on it.

Honestly, I have not had a social security in hand for say 25 years.

What else, besides the DL does need to bring to the 341?

    Thanks !

It's supposed to be your Original SS Card. Read on Chapter 7 board today, one at 341 hearing without SS. Was able to use a paystub with SS# listed.

Go to ssa.gov & you can find the nearest SSA office. You are also able to print the form SS-5 so it will be completed when you arrive at SSA Office.

You will need the original of your Birth Certificate & your Driver's License (I think). Their website will tell you what you need. Said turnaround time is 2 - 4 weeks.

My wife had to apply for a new SS card. Go to the SS website and download the form, fill in out and take it to your nearest SS office with ID and your birth certificate.

She got the new one very fast.

I had to get one. My local SSA office does not use the written form (well, I suppose they would take it) but they ask the required information and enter it directly into their computer. So if you haven't prepared your form, don't worry. They didn't even have any forms at my local office -- everything you want to do is done on the computer with the agent.

The card will come within two weeks.

They will also print out and stamp/sign a "Verification of Social Security Number" letter, which I would assume would be as acceptable as you could get without the actual card.

Yes the letter from the SSA office would work if the card does not arrive in time.

Neato!!! Now all I got to do is find someone to drive me 20 miles to the nearest SSA office. So I just take my original birth certificate and driver's license?

I wish you could request a duplicate SS# either by fax or by email. O Well, some of your wishes are just that - A Wish

                Thanks

Thanks all of you for your help.

** Oh dear, I have to hunt for my birth certificate.

So I guess the SS card and DL are the only forms of identification one would need at the 341?

Correct. Some form of government issued ID (state id, drivers license, passport, etc.) and your SS card.
