I'm fairly new about all this myself, but I will try to answer your questions as best I can. Others will likely be able to give you better answers.
1) I don't think your sister and her son living with you will affect your household size, though they might try to include any money they may pay you as additional income for you. Ask your attorney for more information.
2) I'm not sure if you are allowed to keep a gym membership or not. I would think that maybe if you used your entertainment budget to pay for it you might be able to do that, but then you don't have much for an entertainment budget left. Your attorney should be able to give much more information on these things.
3) This could pose a problem. Again, I highly recommend you talk to an attorney about this and others on this board can probably tell you better than I can. How valuable was this car?
I would also recommend that you do consults with a couple more BK attorneys before you make your decision about which one to go with. Hopefully others will chime in here to help answer your questions. I wish you the best of luck.
