I was wondering what everyone else's experience was with this. I'm about to retain an attorney that I've spoke with twice (a few months ago) and we've e-mailed a few times. However, he won't meet with me in person for the initial consulation until I have an intake form filled out (story of my life - 60 pages long) and a stack of mandatory documents - list is below.
I've been working on getting all this information for quite a long time and my appointment is on Wednesday. So here are my questions.
Is this standard practice for attorneys?
Does all this paperwork get submitted with my bankrupcty or is most of this just to be filed at the attorney's office?

A. INCOME/DEDUCTIONS:
1. _____ Paycheck stubs last 6 months and any year to date. The stubs should
have the following information. If they do not, then we will need documents
showing each of the following.
_____ Retirement or pension plan contributions in the last 6 mo.
_____ Retirement loan currently owed.
_____ Insurance that is being deducted from your paycheck.
_____ Any mandatory or voluntary deduction from your pay.
_____ Any wage garnishment order.
_____ Commissions or bonuses received in last 6 months.
2. _____ Unemployment, child or spousal benefits received in the last 6 months.
3. _____ Soc. Sec. or SS Disability payments received by anyone in household.
4. _____ Pension or retirement benefits received in the last 6 months.
5. _____ Bank interest or stock dividend income received in the last 6 months.

7. _____ If you have your own business, a year-to-date P&L Statement.
8. _____ An itemization of Gross Business and expenses for one year .

B. FINANCIAL:

2. _____ 12 months of monthly bank statements from all financial accounts.
3. _____ Last 2 (two) months statements for every bill you owe.
4. _____ Utility bills for the past 2 months (Just the face page).
5. _____ Last 2 months vehicle and mortgage statements.
6. _____ The last 2 (two) statements for any IRA, 401K or medical savings plan.
7. _____ Education IRA, Education Savings Account or tuition program
8. _____ Any bankruptcy you filed in the last eight years.
9. _____ Copy of any retirement, 401k or pension plan from Plan administrator .

4. _____ Foreclosures, repossessions, garnishments, attachments 1 yr.

F. PERSONAL PROPERTY:

(Secured Transaction is any loan secured by property of any kind.)

G. DIVORCE / SUPPORT

4. _____ Support you owe that is secured by a lien on your home or other

Under the new Bankruptcy Reform, you are required to undergo credit counseling
before you can file bankruptcy and also a financial management course is required prior
