When you are asked to state your name, do you just say your middle initial or your whole middle name?

I think you state it exactly as it appears on your social security card. Remember to bring your original social security card with you to the 341 (and your drivers license). They will not proceed if you don't bring your SS card. And don't forget your folder of paperwork, you will have to fill out a couple pages of paperwork, so you need all your info with you.

I did not state my name exactly as it appears on my ss card and I did not bring any paperwork. I left out my middle name. I also did not have to fill out any paperwork. I did not need to bring any paperwork either. But that's just my experience and I'm a different state.
