In part three of the Statement of Financial Affairs Form I am not clear on whether this is for payment of unsecured debts only or do I list the amounts I have paid in the last 90 days on my car lease and 1st mortgage payments both of which individually total over $600.00.

Yes, sweinc, that is correct...you would list your car lease and the mortgage payment.
