My docket says this form is required of me. I just pulled it up, and I have no secured debt. What do you put on this form, besides your name? There is no "none" to select on any of the sections. I only have unsecured CC debt.

I believe you would write 'none' on the form if there is no box to check, but check with your attorney and insist on NOT paying for the amendment (if there is a filing fee for the correction) since he messed up.
If you filed pro se, you can ask the clerk procedural questions. Indicate you need to file a form that was left out before, ask if there is a fee. Also ask if its correct to just write 'none' regarding secured debts. THe clerk can't give legal advice, but can help with procedural 'how tos'.Get mortgage modified: DONE! 7 months of back interest payments amortized, payment reduced over $200/mo
