Discharged 9/22/09 and Terminated 9/29/09??? Please Help

I just checked Pacer today and this is what it is saying now.
Can someone explain? THANKS!!!

What is the "terminated referring to?"

Is this anything to do with being discharged on 9/22 and a car reaffirmation filed with the court on 9/23? (late?)
