List of forms & questionnaires to be used for Chapter 7?

Can anyone tell me where (on the Internet) I can find sample of the full range of forms and questionnaires used in order to file for Chapter 7? I would like to review them.

Also, before filing, is a filer supposed to complete any kind of background check form that's similar to that used by job applicants, or by people applying for loans from banks?

Do the forms ask about criminal history? What do they ask about tax filing history?

The bankruptcy court in your district should have most of the forms on their website in pdf format. There are no background checks to file bankruptcy.

Do they ask if you've ever been charged with a crime, especially a crime concerning fraud, mispresentation, or larceny? If not, does the US Trustee routinely do a criminal background check as part of the BK process?
