Question on filling out forms

My attorney has a system where I log in online and they input all my infor into the system they have. Well I am trying to get everything all together and have a question or two.

Under the section where I list all my creditors i have just been listing them as I see them on my credit report. None of them are from the original place they came from. For example, I have a American Express card that is not on my credit report but the collection agency that has the account now is. I have just been listing the collection agency, do I need to list where the debt originally came from even if they no longer have anything to do with my account?

I have called all the creditors on my credit report and got all my account info and have listed any accounts that I have that are not on my credit report, I just want to make sure I am doing it right.

You need to list everyone-the original creditor and any collection agency that may have it now.

alright, I assume that I cant get the original creditor info from calling the collection agency that has it now? My debts are three credit cards and the rest medical bills, I know who the original creditors are with the credit cards but with the medical bills it might be a little harder for me to find out because I have been to so many different doctors and hospitals.

Do you have any previous bills they sent you? My attorney required the Original creditor and any collection agencies on my forms.

They haven't sent me bills in a long time, I just email my attorney and he said that I need to find out the original creditor then list the collection agency. I will just call the collection agencies on Monday and find out who the debt is from. The credit cards should be pretty simple to find out and I guess so should all the medical bills. So many forms to fill out, so much stress but somehow I still see a light at the end of my 800 mile long tunnel.
