Credit card disappeared? Not on report/no collections

I had a Citi Mastercard that I let go years ago and it has seemingly just went away. It's not on any credit report nor do they attempt to collect.

I do, however, have an old bill from them... should I list this on my list of creditors anyways?

Also, when asked for account numbers, do I give the collections account number or the original credit card number?

I had some old, old cards from years ago that I had not used in about 20 years, ever since 'Hub and I got married. We always used the main MasterCard. So I did not list them. Nor did I list my expired Sam's Club Membership card.

Sam's notified the BK court that they wished to be included. We wondered why as it had not been renewed since 2004 and we filed Dec. 28, 2007. In talking with the Sam's Club card people, we found that a line of credit was attached to that card that we did not know about. We never used it, and used the card only to gain admittance to the store.

When we explained all of that, and that it had not been renewed since 2004, they notified the court to have their names withdrawn.
