Case Dismissed; Need Motion to Vacate Form

Can someone help me, please! I have filed my Chpt 7 case March 12 pro se. I filed an application to pay my filing fee's in installments. I have made every payment on time, however this last one was 1 day late and now my case has been dismissed. I know I need to file a motion to vacate but don't know where to get the form. Can someone tell me where to go so that I can get this form. I filed in the Eastern Court of Missouri, if that helps. Thanks in advance for any advice/help you can give me.

Does your court have a website with forms? Mine had a generic "Motion" form. I filled out the rest (to vacate dismissal) remember first is notice of motion that needs to be sent to the Trustee as well as the court. You need to file both the notice of motion and the actual motion at the same time. If you do an emergency ex parte motion you can get the dismissal vacated same day. Otherwise there is a two week period for others to object to the motion. Also download a blank "Order" and fill that out the same. If the judge does not have the order, the motion is useless. So I found out the hard way ;)
    12/05/08 - filed pro se
01/27/09 - case dismissed and closed - 02/24/09 - case reopened and dismissal vacated
    04/01/09 - new 341 scheduled
    6/02/09 - DISCHARGED!!!
