The key is, you don't want them calling you at work. You send the letter certified mail return receipt -- so you know that they received it. Being specific that you aren't allowed personal calls at work just makes it very plain and to the point (and specific to the law).
Adding when to reach you just adds another layer of "hey, you have no excuse to call me anywhere else but home because I told you when and where you can contact me".
Again, I would send this certified mail return receipt for your records.
In the end, you want the calls to stop at your place of work, which is why you started this discussion thread.
