Debt included in BK & showing as charged off-can that be?

Ch7 discharge received 7/2005.
I pulled my credit report todday and all the cc debt that we listed in bk showed as 'included in bankruptcy' except for 2 accounts that show 'charged off'. Is this correct, even if we listed them in the paperwork?
Are we liable for the debt? How do I change/fix this?

Note: One account was originally Fleet. Fleet sent us a notice that they were being taken over by Bank of America. In our Bk papers we listed it under Fleet, not BoA. Could this have anything to do with it?
