Go to the BK court site for your district there should be a link there someplace to find what you need, or let me know what state you are in .

Also this should be your lawyer, if the fee was paid to him he needs to file the motion and pay the fee, in order for reconsideration the fee must be paid at the same time that the motion to reconsider is filed, so basically if you file it you will have to pay the fee again if you already paid it to the lawyer.

Pull out your original agreement with the lawyer. Read over it carefully. Then submit to him that you paid him the fee (assuming he doesn't have some scam in there saying its not included) and that he should file the motion and pay the fee. (There is no fee for the motion itself, but you have to pay the fee that was the initial cause of dismissal).

If he fails to comply it might be time to threaten to complain to the states bar. I'd inform him in writing that since he seems to be unwilling to perform his duties as a lawyer that you have no choice but to issue a complaint to the state bar association.

If he still doesn't comply, I'd actually follow suit and file the complaint.
