    does anyone know where I can get a proof of service by form

    There's a sample one on page 107 (and the actual form in Appendix 3) of the Nolo book, do you have that? If not, send me a pm with your email address and I'll scan the pages for you tomorrow afternoon. I am assuming you have Adobe Reader; I could even make it a fillable form if you like. I had no typewriter or even friend with typewriter, and no money to buy the fillable forms, but then I remembered an evil ex-employer had laid a copy of Adobe Acrobat Professional 5 on me, and that's how I did my whole filing. Thank God it was there.

    Just let me know. Back when I was still lurking and studying, you were such an inspiration to me that you were doing it yourself, and just how you were SO determined to do it and get it right! You have no idea how much chutzpah that gave me. Good luck to you!

      yeah i have that book ... I will look it up & thnx and I know what you mean I have adobe pro 8.0 here heeeheee hee think it cost like over 1 grand .. dont ask how I got it... and type writers are expensive and your right no one has one...and thnx- thnx glad I could help anyone do it there selfs (an inspiration) ohhh my thats makes me feel good... .. good luck god bless

        Oh, I'm so glad!

        Yeah, I wouldn't want anyone to ask too very many questions about my copy either...

        I'm still learning it; I never had the time before. The way I did my filing was the free text tool! Little text boxes everywhere! And then I finally figured out the square tool for the check boxes.

        When you said you needed a proof of service is around the time I started playing with form fields, so I went ahead and did one anyway. If you (or anyone else) still need it, I have it. I tried to attach it to this post, but it was 17KB and the limit is apparently 1KB. Oh well.

        I'm not working right now, so it's fun to be able to learn this stuff after all this time of having it and not using it.

        BTW, if I can ask, what did you end up needing it for? I'm filing pro se too, and so far I haven't seen anything I have to mail out myself, until/unless the trustee asks for it. You don't have to answer, I'm just curious.

          luckly my district has fillible forms on there website but if i didnt have adobe 8 I would be able to save the docs..

          I need the proof of service for the statement of intentions .. secured property, to tell secured creditor if i am surrending - redeeming or reafirr.... I am surrending it all they can have it. so I have to send them the statement of intent within 30 days of filing .. which will be dec 13 2007 . and also I have to mail the trustee 2 months of paystub and last year tax return. that is required of everyone in my district.. but yours mite be diff.

            Oh, I see. Yeah, I have nothing to surrender and no secured debts, so I don't have to mail the stmt of intention to anyone, just file it with the bk.

            My constant fear is that I will MISS something just huge that will be an instant dismissal... like when I saw Form 9, I almost cried because I thought I was going to have to do one for every creditor, and then realized that that form was for the courthouse... yeah, I worry too much. But I also feel like an open target because I'm filing pro se and do not even have the pretense of a defense. Thanks for telling me, you made me feel better.

            Oh -- about the forms, the new ones dated 12/07 are not fillable (or weren't when I grabbed them online from the uscourts.gov site) so I had to improvise.

              why did you get your forms from your courts website. not uscourts.gov... maybe they are fillible if they have them... and dont stress too much about doing the wrong thing we all had that just read the question a few times over to understand and you have the nolo soo your good. also you have us.. and we know alot... good luck

                why did you get your forms from your courts website. not uscourts.gov... maybe they are fillible if they have them...
                Nope, I wish. They just have a link to the federal site; they (Middle District of Florida) don't host their own set of forms. Which in a way is cool, because there are no additional local forms to worry about. Most of the old (pre 12/07) forms were fillable, though, it was just my luck I couldn't file by the end of November.

                Y'all are such a blessing, you have no idea. I have learned so much by being here and listening to everyone. Plus, it's well moderated here so there's not a lot of crap to wade through to get to the meaty stuff, like some other forums I've been on. I am so blessed to have found y'all!
