Official Bankruptcy forms are always at the U.S. Bankruptcy Court's website http://www.uscourts.gov/forms/bankruptcy-forms
Your bankruptcy court should be - U.S Bankruptcy Court for the Central District of California. The forms for CACB are located here http://www.cacb.uscourts.gov/forms.
CACB has an excellent web page that lists all the forms required based on the type of bankruptcy that you are filing. Select the appropriate one, such as Chapter 7, and it will tell you which forms are necessary. In fact, they package ALL THE FORMS together and they were last revised December of 2015 (so should have all the recent changes).
That particular court provides excellent guides on how to file as an individual. Be sure to also download the Instructions for your appropriate Bankruptcy Chapter.
If you are ever UNSURE, always contact the Clerk of the Bankruptcy Court and ask a "procedural" question about the forms.
Please also take notice that there are "official forms" and then there are the Local Bankruptcy Rules Forms (LBR Forms). The LBRF forms are also located at the CACB website under http://www.cacb.uscourts.gov/forms/l...cy_rules_forms.
