Are you represented by an attorney? If so, I would definitely call my attorney and give them a heads up that this is winding down and you're ready to do your per-discharge financial management class and file the certificate (if you haven't already done so), and that you have the domestic support obligation (DSO) affidavit as well. Your attorney would typically handle these items when you are represented. If you have a wage deduction order then you should ask your attorney about that as well.
If you are not represented, then you should contact the Trustee's office and ask them what process you file to comply with the procedure to have a Discharge Order entered in your case. If you are not represented, Pro Se, then you should also be checking your District's court web site for information on closing out a Chapter 13 case (maybe under local procedures).
The DSO affidavit is usually before discharge is entered (but probably after your "plan" period is completed). Some District have their own form that you can fill out by hand (or type in with Adobe Acrobat). This type of form is "filed" with the court, meaning that it would be scanned and put into PACER (CM/ECF) as part of the case file. Your attorney, or the Trustee, may have to file the Motion to Vacate Payroll Wage Deduction as well. Procedures are different across Districts as some things the Trustee handles and some things the Debtor's Attorney handles.Chapter 7 (No Asset/Non-Consumer) Filed (Pro Se) 7/08 (converted from Chapter 13 - 2/10)
