Az pro se Have sales contract on home how to get court approval

I thought when the chapter 13 was confirmed in October 13 that it said the estate reverted back to the debtors. However, the title company says that we have to have court approval to sell. We filed pro se. Anyone know where I can find a template for the motion and what documents are required? It seems they just need a copy of the contract, escrow instructions and settlement statements.
