annual income and expense report - post confirmation

Dear forum,

forgive my english. I file chapter 13 alone, with no lawyer. I read on my order to file annual income.

"The debtor(s) shall submit statements of income on an annual basis"

Does some one know what form to file on?

My bankrtupcy court is in Los Angeles, California.

I would think that your tax return would be sufficient to prove your income annually.

I suggest you call the trustee's office and ask what exactly they want. If it is something other than a copy of your tax refund, they may be able to give you a sample of what they want. There probably is no form.
