We had our 341 meeting in early November. Our confirmation hearing was set for early December. We got a paper from the bankruptcy court titled "order confirming plan". It goes on to say "it is ordered: the plan is confirmed" and is signed by the bankruptcy judge. I'm guessing the confirmation was done earlier than they originally told us. Is this normal?

Yes. In some courts, the confirmation hearing doesn't actually take place unless somebody objects to confirmation. My plan was confirmed the same date as the 341.LadyInTheRed is in the black!
