Proof of Service Form?

    Proof of Service Form?

    I've looked everywhere and even called the Trustee who can't even give me the form number. I filed an amended Plan and was just notified I need to send it to all my creditors.

    Anyone know where I can get a "Proof of Service by Mail" form? Is it specific to bankruptcy?

      Essentially, what you need to do is get a friend to mail the items, then certify that they did so.

        You might also want to call the Clerk at Bankruptcy Court to make sure what is expected. I kinda thought that these days, the Clerk of Court actually makes this happen "electronically"...... Give them a call, might save you a headache or two.

          Proof of Service sounds like a series of certified letters, all sent to creditors, showing you informed everyone that your plan is being amended. Probably a form showing that you in fact, notifed the creditors on your matrix. ( I think of Keanu Reeves every time I type Matrix )

          This way they know that they will be receiving more/less depending on how it was amended.

          I can't say this for sure, but it sure sounds close to the mark.

            Many of the BIG Creditors,........... Citi, Chase, BoA,............ Do indeed get notified electronically.

            Creditors can pick their preferred method of Notification and even list their preferred address of contact. The bigger ones typically opt for Electronic Notification where Courts have that capability. Which is most Courts these days.

            I did note in our paperwork on PACER, the Clerk contacted our bigger Creditors both ways. Electronically and the old fashioned way. Court mailing.

            Discharged - 12/2006
            Officially Declared No Asset - 03/2007
            Closed - 04/2007

              Thank you, PETRIFIED -- I had found that form before but wasn't sure if it was "official enough" -- so based on your comments I modified & used it.
